01. Eligibility check — do you qualify?

You qualify if: you filed a Canadian tax return last year, your adjusted family net income is under $90,000, and you don't have access to private dental insurance (through your own employment or a spouse's plan).

02. How to apply

Applications are processed through Service Canada. Apply online at canada.ca/cdcp, by phone at 1-833-537-4342, or in person at a Service Canada office. Have your SIN and last year's tax return ready.

03. Coverage for each family member

CDCP covers individuals, not households. Each qualifying family member must be enrolled separately. Children under 18 can be enrolled by a parent or guardian.

04. What to do after enrollment

05. Annual re-enrollment
