  • No access to private dental insurance — You cannot have dental coverage through an employer, spouse's employer, or private plan
  • Adjusted family net income under $90,000 — Your household income determines your coverage level

Coverage Levels by Income

CDCP coverage is not one-size-fits-all. Your co-payment depends on your income bracket:

  • Under $70,000 — Full coverage, no co-pay required
  • $70,000 – $79,999 — 60% coverage, 40% co-pay

How to Apply for CDCP
