- Canadian resident — You must be a resident of Canada for tax purposes
- Filed a tax return — You must have filed your most recent Canadian income tax return
- No access to private dental insurance — You cannot have dental coverage through an employer, spouse's employer, or private plan
- Adjusted family net income under $90,000 — Your household income determines your coverage level
Coverage Levels by Income
CDCP coverage is not one-size-fits-all. Your co-payment depends on your income bracket:
- Under $70,000 — Full coverage, no co-pay required
- $70,000 – $79,999 — 60% coverage, 40% co-pay
- $80,000 – $89,999 — 40% coverage, 60% co-pay
How to Apply for CDCP
There are three ways to apply:
